Relay not working maybe? is there a separate relay for the fan that you could swap out ? cheap easy and may fix it if your lucky.

Fans are controlled over the CAN via module which selects speed and operation, there is no relay.

Haha, actually I think its a separate LIN connection from the Engine Control Module to Fan Module but a CAN reset is still first logical step. MY15

Probably the fan control module. They are a weak part that is known to fail. My fan was stuck on full but only when the engine was running.
